Primary Care Support

Tapestry Telehealth’s primary care team --- family practitioners, internists, geriatricians, nurse practitioners --- is available on weekdays, on scheduled daily rounds, and on demand to see patients and assess medical issues or changes in medical conditions. Tapestry Telehealth also assigns a dedicated practitioner to each facility. That practitioner gets to know you, your patients and your staff, and integrate seamlessly into you daily care routines.

  • Makes rounds daily alongside a facility’s nursing staff.
